Augusta, Georgia-based national senior care services provider US CareNet has purchased controlling interest in the technology used in its NavCare care management division, forming a new company doing business as HC360 Technologies, the company sdaid.
The newly enhanced platform provides for Chronic Care Management (CCM), Transitional Care Management (TCM) services, and Population Health Management (PHM) services, as well as other care coordination services. In January 2015, the Centers for Medicare and Medicaid Services (CMS) began reimbursing providers who actively manage care delivery for Medicare patients suffering from two or more chronic conditions. These services may be contracted with a third party, such as US CareNet´s NavCare division.
US CareNet formed HC360 Technologies to respond to the ever-evolving healthcare system and its focus on value-based care. Using its more than 30 years of healthcare experience, US CareNet re-engineered and enhanced the platform to more directly address the needs of the chronically ill patient.
NavCare, a US CareNet company, is a care management organization that provides Chronic Care (CCM) and Transitional Care Management (TCM) services to patients as an extension of their physician´s practice using HIPAA compliant CMS certified EHR and proprietary software.
